Given the flexibility of Drupal, any one of these approaches can be viable --
The quiz module can fit in a variety of scenarios, or, depending on the goals of the learning, would not need to be incorporated at all --
It also depends on whether you want to build Drupal out as a SIS, or have Drupal handle the course management piece, or have Drupal and Moodle handle course management alongside an Elgg install that gives users a more learner-centered place to store work/connect with other members of the learning community.
There are a few ways to skin this cat, and the merit of the different approaches will be determined by the context in which the sites will be used.

The only thing I'm missing in Drupal is the part, which is often called "Tests & Surveys".
This should be easily solved with vote.module or -API, or instead of this the quiz.module, but the node-functionality is very rare and the quiz.module is not yet stable (and I have not the skill and time to help here).
All the other eLearning-stuff I'm usually able to realize with the great og.module (subscription, enrollment, ...) and some others.

My vote (and seems to be the only one) went to Drupal quiz module.
After using for almost a year Moodle in a real course, found it far better to use Drupal´s own tools to create a scholar site.
Anyway, I think quiz module should be better than it is now, but it´s a start.
And, as Bill says, It doesn't have to be just one way.
