We have a pressing need at the U.S. Department of Energy for PIV authentication for our customers. The most recent activity on this subject that I can find is from 2012.
Has any Federal agency successfully implemented HSPD-12 authentication with Drupal?

Have you tried OMBMax?
I recommend integrating with MAX auth service as that supports PIV (and lots of other cool stuff) and can be integrated with Drupal. You can also post on the google group, a couple of folks had some suggestions there.

The saml and salml rules modules are your friends.
