quiz 3.x feedback multichoice

We've installed Quiz 6.x-3.x-dev and don't see the "feedback" and correct answer fields when adding a multi-choice question (node/add/multichoice). We've run the upgrade procedure from here: http://contrib.caucusllc.com/quiz-demo/node/16#6

Do we need to get rid of any other legacy quiz tables, nodes etc?

I've attached the screenshots that we see from (node/add/multichoice) and one from another site with the same quiz module version installed where it is working correctly.

Any help would be greatly appreciated!
