I have been looking at the rules from all sides and I just need help from somebody.
I want to add an extra fee for when COD is chosen as payment option. With rules, how would i do that?
I don't see the action "Add line item". Where can I find documentation or a video that explains the underlying philosophy of this system of line items.

I wanted to do something similar and I think it might work for you. I needed to add an amount if the order was COD.
1. In store - configuration - shipping - shipping services - create a new rate I called mine COD rate
2. In Rules - Event: Calculating a shipping rate
Elements: Selected Payment Comparision - Cash On Delivery
Actions: Add a rate for a shipping service to an order - Add COD rate
It worked great for me and I hope you can use it

Yes it worked for me too. I added additional shipping quotes for all shipping services that allow COD and even made them conditional for certain destinations
