Yesterday, our BOA site was down for about 10 min ( 2012-03-20 17:43:53. as reported by Pingdom). During this time we tried to access the site and it was not showing up in the browser and after sometime we tried to reach the site it was redirected to:
http://www.install.php/

After about 10 min, the site was working normally again.

tail /var/log/php/php-fpm-error.log just shows 'rotate'. I guess the old logs were removed and stored elsewhere but I do not know where it is.

We wanted to know why this error occurred and why it suddenly redirected to install.php file so that we can avoid this situation in future.

The same thing has happened with one my sites. Twice. Verifying the site seems to clear the problem. As to why this is happening, I don't know. It seems to be a permission issue... but not 100% sure.

Help

Hey man I want to try to help but without filing something in the issue queue under project barracuda or octopus according to the issue submission guidelines it's next to impossible to debug.

In any event a wild stab in the dark, try migrating the site around 3 times changing its name like so:
attempt 1 > yoursite.com - original
migrate 2 > newsite.com
then > testing.com
and back > yoursite.com

Definitely log an issue in barracuda or octopus issue queue following the instructions over there for submitting issues.

Created the issue
