how to create a form like table structure

Hi, all

i want to create a feedback form for getting feedback from student. i created form using drupal 6 form API. But client should need form view should like attachment png. For that i want to add table td for form field and also options. The options should be in radio buttons. I want some tips to do this form.. any idea..

People are out here to help you, but take little care in properly formatting your question. Was real hard to understand what you meant,. Adding some punctuation could help,..
