Rather than clutter up the Wiki post with self-signed comments, I'm starting this thread to discuss the survey.
First, Thank you, Ms Amye, for moving this topic along.
1] I like the idea of gathering A LOT of information from attendees. In the past when I have filled out surveys, shorter than this one, where my name was also submitted for a drawing for a worthy prize. Not a T-Shirt, or sticker, but an IPOD Nano. This is how I won my 1st and only IPOD; after filling out a survey for the honors society in college. The data collected will be valuable. I believe we need to invest in some serious prizes. Suggestions: Ticket to next NA DrupalCon, donated goods or services from partners like Acquia, Sun, etc. Having the prize on display at a booth would also encourage submissions.
2] The numbering... There are 23 questions listed as #1. Ms Amye mentioned question #16. I saw no #16. I chose not to re-number the questions in the Wiki. But obviously we need better #s at some point.
3] Kiosk in the venue. If we really want to get results we need to set up at least one location in the venue with a terminal where individuals can fill out the survey. This will increase the response. The availability of this terminal would begin at some point on the final day of the event.

Giveaways: We got a whole package of books from O'Reilly to give away. Granted, it was an open source conference, so that's a different market.
But we got more responses because of it. An ipod isn't as helpful anymore, but something like an Android G1/other hacky piece of technology might be good.
Numbering: I think it's just because the wikicode (may it rot in a special place) is wrong.
Question #16 is about: What area of open source are you interested in? It should really be changed towards 'what in this whole wide world of Drupal might you be interested in?'

But seriously. When I travel I have a stash of Wiley Publishing books with me. Their Drupal offerings are growing, so definitely look them up.
I really believe that the "Give Aways" is where we're going to see a tremendous response. Maye we should ask the question in the survey that has offers a grid from which to select there prefered prizes in order.

Apple Store [Come on now! Micro$oft loves Drupal. It's about time that Apple chimed in :-) ]
Is this helpful? Is this discussion adding value to the notion of getting a survey out there and doing our best to see that it gets filled out? If a "Survey Committee" is formed, I'd be happy to join it.

already have a post-drupcon survey
The one we have now gets re-used every year (and for every con) so we can compare responses over time. Were you posting this as a list of additional questions to be added? Can we address them one at a time? What does each new question add? (a lot of this information we have already - as part of drupal.org profiles).
