Permissions(?) Issues

Events happening in the community are now at Drupal community events on www.drupal.org.

Three times now, several items in our primary navigation and other menus have disappeared for site admins, authenticated and anonymous users. (User 1 still seems to be able to see/access these areas of the site.) Even when you navigate directly to the URL, all users aside from User 1 get an Access Denied message. This has happened at least 4 times, and we're not sure of the reason or exactly how its resolved. The first time it happened was after we migrated data from an external database into our site (including users and other nodes). We're unsure of the cause the second time. The third time was immediately after we upped the file upload limit for the admin role and cleared caches. The problem resolved itself 10 minutes later for no apparent reason. The fourth time lasted only 5 seconds (resolved itself again) and was noticed while someone was flushing all caches. Finally, it happened after we were adding/changing a block on our home page.

Any ideas on why this might be happening or how to troubleshoot?

What version of Drupal are you running? This was an issue before version 6.16 was released... If you are running 6.16, then this is scary...

What are your Drupal cache

What are your Drupal cache settings? I've seen this before when memcache wasn't configured properly.

Which access control modules are you using?

Are all the menu items pointing to content or views that are controlled by the user access and mode access systems?

Does this happen to user 1?
