Problem we have: ad will not appear on the page defined in the ad channel under "Display advertisements only on the listed URLs".
Here is the setup we have:
openpublish-2.2-build285
Ad 6.x-2.2
Context 6.x-3.0-beta5
Page: topic-hub/mens-health
Ad group: "Right sidebar top 300x250"
Ad channels: "Everywhere" and "MensHealth" with "topic-hub/mens-health" under "Display advertisements only on the listed URLs".
Context mens-health with "topic-hub/mens-health" under path and with added "ad group: Right sidebar top 300x250"
If published ad belongs to "MensHealth" channel it will not appear even ad block that corresponds "ad group: Right sidebar top 300x250" is in the right place - title will be displayed on the page. Once channel "Everywhere" is added to the same ad it will show up.
Even after we deliberately added to "Everywhere" "Display advertisements on every URL except the listed URLs": "topic-hub/*"
Anyone has idea what is going on here and how to make it work right? Please help.

Do the ads appear if not set to a particular url (eg sitewide or frontpage contexts)?

I don't think that I quite understand what you are asking.
Ads will not appear if set to the sitewide context either until channel "Everywhere" is added.
Few other strange things even after Content permissions have been rebuilt:
1) ad status switches from active to expired every time after I'm saving changes for that ad (but not for those two ads that were created long time before);
2) ad will not display for anonymous even there is permission for it to view ads (only two that created previously will).

However, I am not asking it to do much.
I think I can do whatever I want with context with the existing contexts:

But if I try to get away from this, or start messing with taxonomy, it all goes to hell.
I am running OP 2.2 and I have my banner add and side ad throughout my site. But I have toyed with ads on the various contexts.
The problem I have is that I am using articles for my sports/news and obituaries. I am using taxonomy to differentiate. But the taxonomy doesn't seem to work with context so that I can have a funeral home ad on my obit, and a sports bar on my sports, etc.
I believe this would be best remedied if OP had sections.

And this was the point of understanding that Ad functionality is broken. Idea of channels as conditions: what ad goes in the very same block on the specific page (only two blocks in this case: "ad group: Leaderboard (728 x 90)" and "ad group: Leaderboard right (242 x 90)" can serve numerous ads).
Sure there is a workaround to create new ad group(=ad block) for each of specific ads and use that block only once within specific context, but it's just not supposed to be that way.
Also there is another strange problem with anonymous not seeing ads (this could be a conflict with one of later added modules). Otherwise I love Ad module when it works (was working for me on the previous to OP drupal development without context, so that is why I blame context for problems with ad).
If someone can fix an Ad module to work on OP that would be really great thing.
