Conflict of Interest & Code of Ethics Policies

Section V, 5 of the DrupalCon Inc's 501(c)3 Narrative form says:

See "Conflict of Interest Policy" attached separately."

...but there is no link. I assume that the application to become a Charity/Foundation wouldn't have been accepted without this vital document (also, I'm thinking that saying it's attached when it isn't is a bit bad). So, could someone update that page with the correct link?

Also, a couple of quick questions regarding the policy:
* Is it a zero tolerance policy? (i.e. if DA Board members violate it, are they removed immediately?)
* What is the punishment for violating the policy? (at the very least it should be removal from the Board, but I also personally believe it should disqualify you from sitting on the General Assembly)
* How do community members and/or General Assembly members report violations? How are accusations of misconduct handled?

I also think that as an Open Source community, the DA Board should hold itself to a higher ethical standard than simply avoiding Conflicts of Interest. Here are some resources for "Code of Ethics":
