We seems to have gotten a bunch of intranet-type groups somehow, such as:
http://groups.drupal.org/university-wisconsinmadison
http://groups.drupal.org/yale-university
http://groups.drupal.org/uchicago
And this is promoting other to ask for approval for he same sort of organization-specific group.
How should we handle these? Do we ask them to become broader regional groups?

I agree with this, but not everyone else does.
One idea is to require a certain number of members to get started. People can join a group even before it's public so if we feel these groups are unlikely to attract much attention/traffic we should ask the submitter to get a few friends (5?) to join the group. This is what stackexchange (home of stackoverflow) does when they create new sites, for example.
The longer term need is to find and merge in dead old groups.
